The Six Sigma Forum Award for the Advancement of Six Sigma recognizes outstanding achievements, efforts and contributions from quality professionals that further the Body of Knowledge and practice of the Six Sigma methodology. Given at the ASQ Lean and Six Sigma Conference, this award is presented to individuals who have expanded the use of Six Sigma to drive breakthrough improvements. The next LSS conference is February 28 – March 2, 2021 in Phoenix, Arizona. 

To nominate someone for the award, you must submit a completed nomination form, which demonstrates the nominee has clearly met or exceeded the defined criteria, along with two letters of recommendation. The Six Sigma Forum may also request additional documentation, if required. Self-nomination is not allowed, and previous winners are not eligible.
