Does anyone have references to research on the % of workers who are willing to report quality problems within their organizations?

As willingness to report a problem is a key part of psychological safety, I always look to those surveys. You consistently see about 1 in 8 employees across the surveys, for example this late 2020 Gartner. Which I find fascinating as it was during the height of the pandemic and it still shows about 1 in 8. Didn't even go down much from pre-pandemic levels. Which probably means we are about rock-bottom.
